Does anyone know the name or number of this government form to apply for an access route of pathway across other owners land ? This is an agricultural parcel surrounded by others similar fields.

You'll need to apply for right of way - Form N.252A

Further information on what else you need can be found on page 18 of the Department of Lands & Surveys Citizens Charter.

There are fees to pay; these may be found in their Fees & Charges.

No not wrong Nigel, but the book is insufficient especially if you are going to get it by compulsory methods.

The book is handy as a general reference guide and gives you somewhere to go.

Alls good, I explained to David (as he came to me via your recommendation) the steps and other advice and he is on the case.
